Do you accept my insurance?

I work with some insurance companies in and out of network depending on the plan you have. To determine if you have mental health coverage, the first thing you should do is check with your insurance carrier. Check your coverage carefully and ask:

 

  • Do I have mental health benefits?
  • Do I have Out-of-Network Benefits?
  • What is my deductible and has it been met?
  • How many sessions does my plan cover?
  • Is the therapist I am considering in network?

 

I am a contracted provider with some plans and am able to take out-of-network benefits with other plans. Currently, I am contracted with some Excellus,  Aetna, Cigna, MVP, and Fidelis plans.

What is a Marriage and Family Therapist?

Marriage and Family Therapists (commonly referred to as MFTs or family therapists) are trained and licensed to independently diagnose and treat mental health and relationship problems. Marriage and family therapy is one of the core mental health disciplines and based on the research and theory that mental illness and family problems are best treated in a family context. They conduct, family, couples, individual, group, and family group therapy sessions.

 

 

What are the Rates?

The current rates are listed bellow for the services I offer. These fees are often covered by your insurance company. Please contact me to discuss rates in detail.

 

  • Initial Evaluation: $150
  • 50 Minute therapy sessions (Office and Telemedicine available): $100
  • On Site Consultation and Evaluation: $150 per hour (including travel time)
In homes and in schools when requested and appropriate, such as an IEP or 504 meeting,
or other educational meetings, hospital support for postpartum or other medical concerns.

 

 

How do I Pay?

At this time I accept cash or checks for fees and co-payments. I am also able to accept Visa and Mastercard. (Debit or credit) This also includes HSA and FSA cards.

 

What is the Cancellation Policy?

As the scheduling of an appointment involves the reservation of time specifically for you, you must give at least 24 hours notice to cancel an appointment. If this does not occur, you will be charged a late-cancellation/no-show fee. Fees will be discussed during the intake session. I charge $50 for a missed session and $30 for a late cancellation. Insurance companies do not reimburse for missed sessions. Please note that unless other arrangements are made, the late cancellation fee should be paid at or before your next session. Session fees may be paid in person or by mail.

 

 

Due to the importance of consistency in therapy, three or more late cancellations and/or no-shows may result in the discontinuation of care.

 

 

 

General Directions:


I am located at 243 Center Street in downtown Canandaigua. Turn off of Rt 332 (Main St) onto Beeman St, opposite side of Eddie O'Briens. Take your 2nd left onto Center St. My office is in the white house, second to last from the corner. Park either across street from house or in back. My office is the door in front of the house to left (not the main stairs).
